Patients will be seen by telemedicine for initial evaluation or continuity of care. Internal Medicine Sub-specialty Virtual Clinical Rotation
Program Code:
IM-003Pulm
Clinical Rotation Type: Virtual
Virtual Rotation Type:
Clinical Setting: Medical Clinic
Department: Pulmonary Medicine
- Program Description:
Pulmonary medicine encompasses the diagnosis and treatment of a broad range of disorders that involve not only the respiratory system but may also arise in different organ systems. By virtue of presenting complaints, the pulmonologist must also be well-versed in disorders of the cardiovascular, gastrointestinal, and rheumatologic systems that may mimic pulmonary disease. Many patients with chronic pulmonary disorders are encountered and managed in the clinic, and the management of these patients differs greatly from typical inpatient pulmonary and critical care medicine practices.
The general internist needs to have competency in the initial diagnosis and management of acute pulmonary disorders as well as long-term management of the chronic disease. Disorders such as chronic cough, asthma, COPD, and dyspnea are among the most common seen by general internists and therefore require mastery early in the internist’s career. Interpretation of common studies, such as chest x-rays and spirometry, will be expected of the general internist. More uncommon disorders, such as interstitial lung diseases, cystic fibrosis, and pulmonary vasculitis syndromes, may be encountered only during training or a handful of times during a career; however, having an index of suspicion is critical for timely referral to a specialist. Finally, working knowledge of medications used in the care of patients with pulmonary disorders is critical for the internist, since these physicians often coordinate patient care and will undoubtedly be asked to assess for potential side effects. - Program Goals:
• Interpret PFTs and ABGs, and chest x-rays, and understand the relative diagnostic features of V/Q scans and chest CT.
• Recognize and respond to signs of impending respiratory failure.
• Understand and describe the initial evaluation for sleep disorders, solitary lung nodule, pleural effusion, interstitial pneumonia, latent tuberculosis, COPD, asthma, and pneumonia.
• Rotating students/graduates will participate in the reading and interpretation of sleep studies.
• Evaluate and manage obstructive, restrictive, and thromboembolic pulmonary diseases.
• Manage asthma at various levels of severity.
• Recognize clinical presentations of exercise-induced asthma, cystic fibrosis, and pulmonary hypertension.
• Understand the action and pharmacology of common pulmonary medications including bronchodilators, steroids, other anti-inflammatory agents, and ancillary therapies.
• Understand the use of invasive and noninvasive (including CPAP and NIPPV) ventilation. - Clinical Competencies:
• Complete a comprehensive pulmonary consultation including identification, chief complaint, history of present illness, past history, review of systems, personal, and social history.
• Developing a framework of assessing physical examination (via telemedicine) with a particular focus on the pulmonary examination.Rotation Block: Four Weeks
